I want to create a river between two cities like there is in the real world. If I want to connect to the other city by bridges I guess an island would be needed to connect like a road connection. Any idea how many tiles the length and width would be needed to make the connection work? I guess a peninsula would work also just still need the length and width so I do not get the red can not place warning.

One or two tiles would be needed to make the connection + bridge (I think it will depend on the bridge), though i can be wrong on that one.
On the other tile the connection will create a 3/4 long road piece so be warry of that when you plan your island.
I'll take a quiet life... A handshake of carbon monoxide.

I my experience, it would require four tiles total, two in the ending of the RHW6S and above bridge for a starter piece and two tiles for the beginning of the next bridge for a small slab of island

Or, you can use the Diagonal Bridge Enabler.
