There are a couple of articles in the OmniMaps - Mapping and Terraforming Information section of the Simtropolis Omnibus that you might want to read through:

Maps: The Sea Level Problem with USGS Data

MallowTheCloud started a tutorial on making maps from DEM data but I don't believe it is complete.  Nonetheless, you might be able to get some ideas from it:

Making Accurate-Scale Real-World Maps

Finally, you might want to look through the Mapping Applications and Mapping Tools article for DEM sources and related software.
